HopeConnect is a behavioral health referral coordination network. It connects the organizations that send and receive referrals, tracks each referral through its lifecycle, and closes the loop back to the sender when the patient is seen.

No. An EHR documents care inside one organization. HopeConnect coordinates referrals between organizations, and it runs alongside whatever record system you already use. Nothing gets ripped out.

No. A directory is a list somebody assembled and nobody maintains. In HopeConnect each provider maintains their own Passport, including plans accepted, populations served, credential status, and real availability, because that is what gets them referrals that fit.

No. HopeConnect is coordination infrastructure. It organizes, flags, and routes, and it never diagnoses, triages, assigns acuity, or decides who a patient should see. Those judgements stay with clinicians.

Individual providers join free during the Founding Network, with no per-referral fees. Organization pilots are scoped to referral volume, and we give you the number before you commit to anything.

Only what a referral needs: the referral and its events, documents attached to it under consent, messages on the case, appointments, and eligibility checks. There is no chart, no progress notes, no medication list, and no diagnosis history.

Consent is a blocking gate in the workflow. Where Part 2 applies, a referral cannot be sent until consent is verified, and records governed by Part 2 do not move without it.

It means the organization that made the referral learns the patient was seen, based on a recorded event rather than an assumption. Referral sent, referral accepted, and appointment booked can all be true while the patient still never receives care.

On explicit criteria: clinical focus, population, insurance, care setting, availability, and credential status. Every match shows which criteria were met, so a coordinator can defend the choice and a provider knows why they appeared.

It is labeled. Where a provider's plan participation or other detail was inferred rather than stated, HopeConnect flags it and asks you to confirm before sending, rather than presenting a guess as a fact.

Not yet, and we will not invent them. HopeConnect is pre-launch and building its founding network in Tennessee. When we have outcomes measured on real referrals, we will publish the real numbers.

We are building the founding network in Tennessee first, because a referral network is only useful where it is dense enough that referrals actually find someone. Statewide launch follows the founding pilots.
