This policy covers two different things: information collected through this website, and protected health information handled through the HopeConnect platform on behalf of healthcare organizations. They are governed differently, and this policy keeps them separate.
Scope
HopeConnect Health ("HopeConnect", "we") operates this website and the HopeConnect referral coordination platform. This policy explains what we collect in each context.
Two distinct roles. On this website, we act on our own behalf. In the platform, we handle protected health information as a business associate on behalf of healthcare organizations, and their agreements with us and with their patients govern that information. Where this policy and an executed business associate agreement differ regarding protected health information, the agreement controls.
Information collected through this website
This website sets no cookies of its own and runs no analytics or advertising trackers. See our cookie policy for detail.
We collect information you choose to give us:
- Application form. If you apply to the Founding Network, we collect what you enter, which may include your name, email address, organization, role, and details about your practice or referral volume.
- Email. If you write to us, we hold your message and address in order to reply.
We use this to respond to you, to assess and set up founding network participation, and to keep you informed about launch. We do not sell it, and we do not share it with advertisers.
Please do not send patient information through this website or by email. Neither is an appropriate channel for it, and we do not need it in order to answer a question.
Information handled in the HopeConnect platform
Organizations using the platform submit information necessary to coordinate a referral. The platform is deliberately narrow in what it holds: the referral and its events, documents attached to it under consent, messages about it, appointments, and eligibility checks. It does not hold a patient chart, progress notes, medication lists, diagnosis history, or treatment plans.
We process this information to provide the service to the organization, under their instruction and under our agreement with them. Where 42 CFR Part 2 applies, disclosure requires verified consent and the platform enforces that as a blocking gate.
How information is shared
- Between participating organizations, to the extent required to coordinate a specific referral, scoped by role and governed by consent.
- With service providers who host or support the platform, under obligations consistent with our commitments, including business associate obligations where applicable.
- Where legally required, subject to the restrictions that apply, including the stricter limits under 42 CFR Part 2.
We do not sell personal information, and we do not use protected health information for advertising.
Counsel to confirm
Named subprocessors and hosting locations, international transfer position if any, and the precise legal-process response commitments. These depend on your actual infrastructure and contracts.
Your choices and rights
For information you gave us through this website, you can ask us to access, correct, or delete it by writing to info@hopeconnecthealth.com. You can ask to stop receiving email from us at any time.
For information held in the platform on behalf of a healthcare organization, requests should go to that organization, which controls the record. We will support them in responding.

State-specific rights language, including whether Tennessee or other state privacy statutes apply to your operations, response timeframes, verification procedure, and any appeal process required.
Retention
We keep website enquiry information for as long as needed to respond and to administer founding network participation. Platform information is retained under the agreement with the organization concerned, including terms for return or deletion when that agreement ends.
